PLEASE READ CAREFULLY BEFORE SUBMITTING A

DIPLOMA REORDER REQUEST

Steps for reordering your North Monterey County High School Diploma:

1.)Complete all sections in the form below

2.) Upload a copy of a valid Photo ID then submit this form

3.) Payment: $52

  • Pay in person at the Coordinator's Office at NMCHS, cash or money order made out to NMCHS

  • Send your payment via US Mail, money order only made out to NMCHS, to address:

No Credit or Debit card payments accepted at this time.

NOTE: Once fee is paid it will take 4-6 weeks for processing (important: diploma re-orders will have current Admin signatures).

IMPORTANT - Due to FERPA privacy laws: If you are over the age of 18 and need someone other than yourself to pick up your Diploma, you must indicate their full legal name below (Release To). Copies of both valid photo ID's will be required prior to releasing your Diploma.

REQUESTS ARE NOT PROCESSED ON DAYS WHEN THE SCHOOL IS CLOSED.
