Transcript/Document and Diploma Re-Order Forms
PLEASE READ CAREFULLY BEFORE SUBMITTING A TRANSCRIPT/DOCUMENT REQUEST
Steps for Requesting your North Monterey County High School Transcript:
- Pay in person at the Coordinator's Office at NMCHS, cash or money order made out to NMCHS
- Send your payment via US Mail, money order only made out to NMCHS, to address:
13990 Castroville Blvd.
Castroville, CA 95012
IMPORTANT - Due to FERPA privacy laws: If you are over the age of 18 and need someone other than yourself to pick up your transcript, you must indicate their full legal name below (Release To). Copies of both valid photo ID's will be required prior to releasing your transcript.
Transcript requests will be processed by our Student Transcript/Data Specialist within 5 business days.
REQUESTS ARE NOT PROCESSED ON DAYS WHEN THE SCHOOL IS CLOSED.

Steps for reordering your North Monterey County High School Diploma:

Pay in person at the Coordinator's Office at NMCHS, cash or money order made out to NMCHS
- Send your payment via US Mail, money order only made out to NMCHS, to address:
13990 Castroville Blvd.
Castroville, CA 95012
NOTE: Once fee is paid it will take 4-6 weeks for processing (important: diploma re-orders will have current Admin signatures).
